Note-taking services are provided as an accommodation to ODS students who have been granted this accommodation by their program coordinator. Students utilizing this accommodation must abide by ODS's note-taking services guidelines to ensure timely and efficient provision of these services.

Please provide the following information, which will be used to secure note-takers for the courses you indicate below. Please list all classes for which you will need note-taking services. Incomplete course information will result in a delay of services. Please check your preference for electronic typed notes or handwritten notes for each class. ODS will make reasonable efforts to accommodate students' preferences regarding the type of notes provided, but cannot guarantee that acceptable notes in the students' preferred medium will be available.
