Information about access to Columbia Health's programs and services while you are participating in your summer program at Columbia University.

Medical Services

Students participating in Summer Program at Columbia University can access Medical Services for urgent medical concerns; we are located on the fourth floor of John Jay Hall.

Immunization Compliance

Columbia University requires that all participating students provide documentation of immunity to measles, mumps, and rubella in accordance with New York state public health law. The process for documenting immunity is described on page two of the immunization form.

  • In person/drop-off – John Jay Hall, 3rd fl. Please check our hours of operation. The form drop-off box is on the 3rd floor landing, no swipe-in required. Students can drop-off anytime.

We are unable to accept health forms or immunization documentation via email for privacy and security reasons.

It is important that you provide Columbia Health with accurate and complete health-related information. This questionnaire will prepare us to address any special medical problems you may have.
