All incoming students are expected to complete the online Columbia Health Pre-Arrival Questionnaire on the Patient Portal (Medical Clearances section) prior to their arrival on campus. It includes medical information that is pertinent to ongoing care, particularly around current treatment. Columbia Health does not share this information, or any other information about your health, without your written permission unless required by law.

How Information is Used
