Final Exam Accommodation Request Form
April 29, 2026 Update: Final exam accommodation requests are no longer being accepted via this form.
If you have a last-minute request, please email [email protected] with the subject line “Last-Minute Testing Accommodation Request” and include your name, UNI, phone number, course name, professor name and quiz/exam date in your email. We will confirm availability to schedule your exam at DS after reviewing your request. Please allow for at least 1-2 business days.
By 7 p.m. on Friday, April 10, DS-registered students will receive an email containing their exams scheduled with our office during the final exam period (May 8 - May 15)
Exams at DS are scheduled in accordance with the exam dates/times assigned by the registrar. Please note that exams with DS will be scheduled at 9 a.m., 1 p.m., and 4 p.m. All 7 p.m. exams will be scheduled with DS at 4 p.m.
Please review your exam schedule carefully to ensure (1) all of your final exams are scheduled and (2) scheduled at the correct dates/times.
Final Exam Scheduling Options
Last-minute request approvals during final exam period are very limited.
The deadline for scheduling any additional final exams is Friday, April 17 at 10 a.m. We cannot guarantee access to exam accommodations for exams requested after Friday, April 17.

If your professor scheduled your final exam during reading days
- Email [email protected] with the subject line “Reading Days Final Exam Scheduling Request.”
- Include the Course Name, Number, and Professor for the exam.
- List the date/time your professor has scheduled their exam.
- Share a screenshot of the email or Courseworks announcement where this information was announced.
Note: Our reading day proctoring is very limited so your exam with our office may be scheduled at an alternate date/time.
Overlapping Finals/Exam Conflicts/3 Exams per Day
For students with any time conflicts with exams (exams that overlap given your extended time accommodation, and the University’s defined exam schedule).
- Email [email protected] with the subject line “Final Exam Conflicts.”
- List the dates/times of conflicting exams.
- Include the Course Name, Number, and Professor for each exam.
Religious Observance Conflicts
For students who are Sabbath observant and receive time-based testing accommodations, exams scheduled at 1 p.m. or 4 p.m. on Friday, May 8 will conflict with the Sabbath.
- Email [email protected] with the subject line “Final Exam Conflicts.”
- Specify that this is a religious observance conflict.
- List the dates/times of exams that conflict with the religious observance.
- Include the Course Name, Number, and Professor for each exam.
