Allergen Immunotherapy Administration Form

Medical Services administers allergy immunotherapy injections, a.k.a. “allergy shots” for patients with a current written order (within the past 12 months) from a New York State-licensed prescribing allergist.
Allergy testing is not available at Medical Services. We do not have an allergist on staff.
Eligibility
Students who have paid the full-time Columbia Health and Related Services Fee and spouse/domestic partner dependents on the Columbia Plan who have a a valid prescription (within the past 12 months) and treatment plan from a New York State-licensed allergist.
How to Get Started with Allergy Injections
Prior to receiving injections at Columbia Health
Have your New York State-licensed allergist* complete and submit the Columbia Health Allergen Immunotherapy Administration Form.
Submission options:
- Bring the completed form to your initial allergy care visit to Medical Services.
- Upload the completed form to the “Downloadable Forms” section of the Patient Portal.
- Your provider can fax the completed form to 212-851-2477.
Note: the allergist must administer all your initial injections.
Initial allergy care visit
- Call 212-854-7426 to schedule an initial allergy care visit with your primary care provider at Medical Services. Note: Online scheduling is not available for the allergy care visit.
- Review your medical history, treatment plan, and other considerations with your provider. If Medical Services has received the Allergen Immunotherapy Administration Form, they will discuss this with you at this visit.
- Schedule a nursing appointment for your allergen injection visits.
Ongoing allergen injection visits
Please bring your serum supply at your first visit, or subsequent visits where you have an updated dose. Vials should be labeled clearly with your legal name, date of birth, vial concentration, and expiration date. Medical Services does not accept serum mailed directly to our facilities.
- For safety, you must wait 30 minutes at Medical Services after the injection. The nurse will not proceed with the allergen injection if you cannot stay for the full 30 minutes.
- Students with significant systemic reactions must be evaluated by their allergist before further injections.

Students on the Columbia Student Health Insurance Plan: Schedule an appointment with your primary care provider at Medical Services to establish care and receive the required referral prior to meeting with an allergist
Students on other insurance: Find an in-network allergist licensed in New York State.
