Immunization Documentation Upload Instructions
Step 1: Complete the Pre-Registration Immunization Form and/or collect documentation for each* required vaccine. Complete the meningitis decision form (see instructions for certifying your decision) on the Patient Portal.
Step 3: From the Patient Portal home page, select ‘Medical Clearances’ from the left-hand menu.
Step 4: Upload the immunization documentation by clicking ‘Update’ next to “Upload Immunization Files” (bottom row under Items required for clearance).
After the immunization document(s) is successfully uploaded and in review, you can view your document by clicking on the image. Upload additional documents by clicking ‘Update’ again.
Step 5: Return to the ‘Medical Clearances’ menu. Enter immunization dates for Measles, Mumps, Rubella and Meningitis*) by clicking ‘Update’ next to each vaccine. Note: this step needs to be repeated for each disease individually.
What happens next:
Once the (1) immunization documentation is uploaded and (2) dates of immunization are entered for each* required vaccine, your file will be awaiting review. Please allow up to 15 business days for processing.
If further action is required, you will receive an email to your Columbia email account with instructions.
You will receive a confirmation email once your record has been processed and the hold has been lifted. You may also check your status online to verify your hold has been removed:
- Morningside/Manhattanville students: SSOL
- Teachers College students: My TC Portal. Go to the Student Account section and click the link for “View Holds”
*required vaccines: Documentation of meningitis ACWY is only required if you certified that you received the meningitis ACWY within the past 10 years.
