Quiz Accommodation Request Form
The deadlines to submit requests are:
- Quizzes between October 5 and October 30: Friday, September 25 at 10 a.m.
- Quizzes between November 4 and December 14: Thursday, October 29 at 10 a.m.
Request guidelines
- Please use this form to submit requests for in-person quizzes only.
- The webforms will not accept requests for quizzes with less than 1 week's notice.
- All requests submitted after the deadlines above are subject to availability in exam rooms and proctor schedules.
- For any questions about exam or quiz accommodations, email: [email protected].
Please review your quiz schedule carefully to ensure all of your quizzes are scheduled and scheduled at the correct dates/times. If you have a conflict, please email [email protected] immediately with the subject line “quiz conflict.”
Do not use this form:
- For midterm exams for any course, please submit a Testing Accommodation Request.
- For online and take-home exams and quizzes. Instead, refer to the Testing Accommodations section of the Academic and Campus Accommodations page.
- For graduate exams. Instead, contact the appropriate liaison.
- To schedule placement exams or deferred exams. Instead, email [email protected].
- If you are a Barnard student. Instead, contact CARDS.
- For JTS courses, contact your Dean.
- If you have an urgent, last-minute request and the webform has closed for your quiz date, please email [email protected] with the subject line “Last-Minute Quiz Accommodation Request” with the subject line “Last-Minute Quiz Accommodation Request” and include your name, UNI, phone number, course name, professor name, and quiz date in your email.
The form has been closed. Please contact DS ([email protected]) if you would like to schedule any quizzes.
Reminder, this form is only for quizzes.
- Accommodation letters are requested using the accommodation letter request form.
- If you are eligible for Note-taking Accommodations, you will still need to submit your request via the Note-taking Accommodations request form.You should allow for 1-2 weeks for this accommodation to be coordinated.
- For exams, please must submit a Testing Accommodation Request.
