Testing Accommodation Request Form

The deadlines to submit requests are: 

• Exams between October 5 and October 30: Friday, October 2 at 10 a.m.
• Exams between November 4 and December 14: Thursday, October 29 at 10 a.m.

Please use this form to submit requests for in-person exams only.

Do not use this form:

• For exams that do not yet have an announced date. Refer to guidance in the Academic and Campus Accommodations FAQ.
• For online and take-home exams and quizzes. Instead, refer to the Testing Accommodations section of the Academic and Campus Accommodations page.
• For graduate exams. Instead, contact the appropriate liaison.
• To schedule placement exams or deferred exams. Instead, email [email protected].
• If you are a Barnard student. Instead, contact CARDS.
• For JTS courses, contact your Dean.
• If you have an urgent last-minute request and the webform has closed for your exam date, please email [email protected] with the subject line “Last-Minute Testing Accommodation Request” and include your name, UNI, phone number, course name, professor name and exam date in your email.

Reminder, this form is only for exam requests.

• Accommodation letters are requested using the accommodation letter request form.
• If you are eligible for Note-taking Accommodations, you will still need to submit your request via the Note-taking Accommodation request form.You should allow for 1-2 weeks for this accommodation to be coordinated.
• For quizzes, please must submit a Quiz Accommodation Request.
