How to Request an Immunization Exemption

Please complete the form(s) and submit, with all required documentation, via the Columbia Health Patient Portal. Submit the entire packet of required and fully completed items at one time. Partial or incomplete submissions are not reviewed and will result in significant delays.
All exemption requests are due on admission or no later than 30 days before registration. Requests submitted late cannot be expedited and may impact the ability to move into on-campus housing, access campus, or participate in campus activities (including new student orientation).
- Go to secure.health.columbia.edu and log in with your UNI.
- Select "Medical Clearances" from the left-hand menu.
- Select "Update" next to Upload Immunization Files.
- Select "Upload" and attach the form and required documentation. Accepted formats are .gif, .jpg, .png, and .pdf. There is a 4 MB file size limit
- If the item is legible, click the "Looks Good" button; then click the "Save" button.
- After you upload and submit your entry, the University will review your request. Allow at least 15 business days for processing.

- Read the pre-registration instructions requirements.
- Go to portal.studenthealth.cuimc.columbia.edu and log in with your UNI.
- Follow the instructions on the portal. Note that this immunization documentation is not listed on the current pre-registration forms.
